Dear Collegiate Parents and Guardians,

As a reminder, we have two forms that are sent home, reviewed and completed. We ask that you complete this electronically by Friday, October 14, 2022. Only one parent/guardian needs to complete step 2, for each of their Collegiate students.

1. Please review the Student Parent Handbook for 2022-2023 school year. They can found here in four different languages:

2. Please review and complete the sign off forms, located below. The first page is required, and acknowledges you read and reviewed the handbook. The second/third page is optional, and only required to complete if you would like to opt your child out of being included in the yearbooks, etc. and also not publishing student photos or student work. You should complete this form for each of your Collegiate students.

The Student/Parent Handbook is a very important and informative document that contains everything you need to know to ensure a smooth and successful academic year. Please read it thoroughly and sign as acknowledgement that you have read and understand the policies and information stated in the Collegiate Charter School of Lowell Student/ Parent Handbook, which includes descriptions of the school's expectations in such areas as attendance, uniforms, behavior, parent involvement, policies, forms and health-related areas and an explanation of academic and emergency procedures.
